fa مقايسه اثر دو دوز مختلف سم ‌بوتوليسم در درمان شقاق مزمن مقعد Comparison of the Effects of Two Different Doses of Botulinum Toxin in the Treatment of Chronic Anal Fissure داخلي Internal Medicine پژوهشي Research <roundrect id="_x0000_s1026" style="MARGIN-TOP: 0.55pt; Z-INDEX: -1; LEFT: 0px; MARGIN-LEFT: 10.7pt; WIDTH: 467.65pt; TEXT-INDENT: 0px; POSITION: absolute; HEIGHT: 524.15pt; TEXT-ALIGN: left" arcsize="734f" /><textbox /><table cellspacing="0" cellpadding="0" width="100%"><tbody><tr><td style="BORDER-RIGHT: #ece9d8; BORDER-TOP: #ece9d8; BORDER-LEFT: #ece9d8; BORDER-BOTTOM: #ece9d8; BACKGROUND-COLOR: transparent"><div> </div></td></tr></tbody></table></textbox /><wrap anchorx="page" /></roundrect />خلاصهسابقه و هدف: شقاق مزمن مقعد بيماري شايع ناحيه انوركتال مي‌باشد. بررسي‌هاي مانومتريك افزايش فشار در ناحيه اسفنكتر داخلي مقعد را عامل جلوگيري از بهبودي شقاق مي‌داند. بنابراين درمان بر روي كاهش فشار اسفنكتر مقعد متمركز شده است. يكي از روش‌هاي درماني تزريق سم بوتوليسم در اسفكتر مقعد مي‌باشد اما دوز ايده‌آل آن هنوز مشخص نشده است. هدف بررسي حاضر مقايسه تأثير دو دوز مختلف سم ‌بوتوليسم بر روي ميزان بهبودي و عوارض درمان مي‏باشد.مواد و روش‌ها: تعداد 70 بيمار مبتلا به شقاق مزمن مقعد كه سابقه بيماري بيش از 3 ماه داشتند و در معاينه مقعد علايم مزمن شدن به صورت پيدايش skin tag و يا ديده شدن فيبرهاي عضله داخلي مقعد در كف زخم داشتند وارد مطالعه شدند. بيماران به طور تصادفي به دو گروه تقسيم شدند، گروه اول (I) توكسين‌بوتوليسم 80 واحد و گروه دوم (II) 200 واحد دريافت داشتند. بيماران به مدت يك سال پيگيري شدند. بهبودي علايم و يا پيدايش اسكار ناشي از بهبودي زخم به عنوان پاسخ درماني مناسب منظور شد.يافته‌ها: 38 بيمار مرد و 32 بيمار زن بودند متوسط سني بيماران 2/11±3/35 سال، متوسط طول بيماري 15/4±29/10 ماه بود 85/82% بيماران شقاق در ناحيه خلفي مقعد داشتند. شايع‌ترين علامت، درد هنگام دفع مدفوع بود . يك بيمار از گروه با دوز كم و در چهار بيمار از گروه با دوز بالا بي‌اختياري اسفنكتر مقعد براي گاز بوجود آمد كه در چهار بيمار بي‌اختياري اسفنكتر مقعد قبل از 4 هفته، و يك بيمار قبل از 8 هفته بهبودي كامل يافت و هيچ بيماري عارضه دايمي نداشت.62 بيمار مطالعه را به پايان رسانيدند. و هر دو گروه 4 نفر براي پيگيري مراجعه نكردند بعد از يك هفته تنها در گروه دوز بالا (II) 4بيمار بهبودي شقاق داشتند و هيچ كدام از بيماران گروه دوز كم (I) بهبودي نداشتند. بعد از دو هفته 20% بيماران گروه I و 50% بيماران گروه II بهبودي داشتند. درپايان 8 هفته 80% بيماران گروه اول و 6/90% بيماران گروه دو بهبودي داشتند. دو بيمار در گروه دوز كم (I) قبل از پايان شش ماه دچار عود سريع شدند وليكن در گروه دوز بالا (II) هيچ بيمار دچار عود سريع نشد. بعد از پايان مطالعه 22 بيمار از گروه دوز كم و 29 بيمار از گروه دوز زياد بهبودي كامل داشتند. بعد از پايان مطالعه، 6 بيمار در گره دوز كم و3 بيمار درگروه دوز زياد عدم بهبودي داشتند كه 3 بيمار در گروه I و دو بيمار در گروه II مبتلا به شقاق قدامي بودند. بعد از يك سال پيگيري چهار بيمار در گروه يك و دو بيمار در گروه دو عود بيماري داشتند.نتيجه‌گيري :به نظر مي‌رسد افزايش دوز توكسين‌بوتوليسم باعث افزايش پاسخ درمان در بيماران مبتلا به شقاق مقعد مي‌شود اين افزايش دوز همراه با عوارض جانبي دايم نيست.واژه‌هاي كليدي: شقاق مزمن مقعد، سم ‌بوتوليسم، آنوركتال Comparison of the Effects of Two Different Doses of Botulinum Toxin in the Treatment of Chronic Anal Fissure   H. According to manometric studies, the increase of anal sphincter pressure is the most common cause of chronic anal fissure. Therefore, treatment is focused on decreasing the anal sphincter pressure. The injection of botulinum toxin into anal sphincter is one of the treatments, but its ideal dose has not been identified yet. The aim of the study was to compare the effects of the two different doses of toxin on fissure healing rate, and also assessement of the complication of the treatment. Materials and Methods: 70 patients with history of more than 3 months anal problem, with signs of chronic anal problems, such as skin tag and or visible internal anal sphincter muscle fiber at the base of ulcer were enrolled. Results: 38 patients were men and 32 patients were women. Mean age was 35.3+11.2 years, and average length of illness was 10.29+4.15 months. The results showed 82.85% of patients had posterior anal fissure. The most common symptom was pain with defecation. In each group, four patients were lost from the study and 62 patients completed the study. One of the patients from the low dose group (I) and 4 patients from the high dose group (II) had gas incontinence. After one week, only 4 patients from the high dose group (II) had recovery, but no patient of the low dose group (I) had recovery. After two weeks, 20% of the patients in group I and 50% of the patients in group (II) had recovery. After 8 weeks, 80% of the patients in the group I and 90.6% of group II had recovery (P=0.29). Two patients in group I had rapid recurrence before six months. There wasn't any rapid recurrence in group II .At the end of the study, 22 patients from group I and 29 patients from group II had complete recovery. Six patients from group I and 3 from group II showed no response to the treatment, and most of them had chronic anterior part fissure. Four patients in group I and 2 patients in group II had recurrence after the one year follow-up. fa اثر تزريق روغن كنجد در ناحيه CA1 هيپوكامپ بريادگيري و حافظه فضايي و تداخل آن با استروييدهاي جنسي در موش‌هاي صحرايي نر The Effect of Sesame Oil Injection into CA1 Area of Hippocampus on Spatial Learning and Memory, and its Interaction with Sexual Steroids in Adult Male Rats فيزيولوژي Physiology پژوهشي Research خلاصهسابقه و هدف: روغن كنجد يكي از روغن‌هاي گياهي است كه استفاده آن در طب سنتي قدمت ديرينه دارد. شواهد نشان مي‌دهند كه اسيدهاي چرب غيراشباع فرآيندهاي يادگيري و حافظه را تعديل مي‌نمايند. در اين مطالعه، اثر تزريق روغن كنجد در ناحيه CA1 هيپوكامپ بر يادگيري و حافظه فضايي موش‌هاي صحرايي نر بالغ (سالم و اخته) مورد بررسي قرار گرفت.مواد و روش‌ها: حيوان‌ها به طور تصادفي به 6 گروه (در هر گروه 7 سر) تقسيم شدند ، 3 گروه سالم و 3 گروه اخته شده كه همه گروه‌ها در ناحيه CA1‌ هيپوكامپ كانول‌گذاري ‌شدند و عبارت بودند از: گروه‌هاي كنترل سالم و اخته شده كه هيچ‌گونه تزريقي نداشتند، گروه‌هاي شاهد سالم و اخته شده كه سرم فيزيولوژي و گروه‌هاي سالم و اخته روغن كنجد كه به مدت 5 روز هر روز بلافاصله قبل از آموزش به ترتيب 5/0ميكروليتر سرم فيزيولوژي و روغن كنجد به صورت دو طرفه در ناحيه CA1 هيپوكامپ دريافت مي‌كردند، و سپس به مدت 5 روز و هر روز 30 بار توسط ماز Y آموزش مي‌ديدند. همه گروه‌ها پس از يك‌ ماه مجدداً براي آزمون حافظه يك جلسه در دستگاه قرار مي‌گرفتند.يافته‌ها: تجزيه و تحليل آماري نشان مي‌دهد كه روغن كنجد در حضور و عدم حضور غدد جنسي باعث افزايش يادگيري شده، اما در حضور غدد جنسي در ميزان حافظه، يك ماه بعد، تغييري ايجاد نكرده است. هم‌چنين عدم حضور غدد جنسي تأثيري در ميزان حافظه ناشي از روغن كنجد در مقايسه با حضور غدد جنسي نداشته است.نتيجه‌گيري: بنابراين به نظر مي‌رسد كه روغن كنجد باعث بهبود فرآيند يادگيري مي‌گردد، علاوه بر اين احتمالاً اين روغن براي اعمال اثرات بهبود دهندگي خود بر حافظه با هورمون‌هاي جنسي تداخل عمل دارد.<rect id="_x0000_s1026" style="MARGIN-TOP: 106.35pt; Z-INDEX: 1; LEFT: 0px; MARGIN-LEFT: -5.6pt; WIDTH: 159.95pt; POSITION: absolute; HEIGHT: 24.5pt; TEXT-ALIGN: left" strokecolor="white" /><wrap anchorx="page" /></wrap /></rect />واژه‌هاي كليدي: يادگيري و حافظه فضايي، روغن كنجد، ناحيهCA1 ‌ هيپوكامپ، اخته كردن  The Effect of Sesame Oil Injection into CA1 Area of Hippocampus on Spatial Learning and Memory, and its Interaction with Sexual Steroids in Adult Male Rats  R. Evidences indicate that unsaturated fatty acids can modulate learning and memory. In this study the effect of intrahippocampal injection of sesame oil on spatial learning and memory in N-MARI intact and castrated adult male rats was investigated. Materials and Methods: The animals were microcannulated, and divided into 3 intact and 3 castrated groups (n=7 in each group). The intact and castrated control groups (no injection), the castrated and intact saline sham and sesame oil. Test groups received bilaterally, 0.5 m l saline or sesame oil into the CA1 region of hippocampus every day for 5 days immediately before the training. Then each rat was trained 30 times every day, for a total of 5 days with Y-maze. After a month, all the groups were tested (one session) for memory test. Results: Statistical analysis of data showed that sesame oil increases learning in both castrared and intact groups, but it did not change the spatial memory in intact animals. Also it did not prevent memory decline in castrated animals. Conclusion: It seems that sesame oil increases learning task. On the other hand, it is probable that sesame oil has an interaction with sexual hormones to exert its enhancement effect on learning task . fa نقش عضلات قدامي ساق پا در كنترل حركتي عضلات چهار سرراني در وضعيت‌هاي مختلف مفاصل زانو و ران در انسان The Role of Pretibial Muscles in the Motor Control of Quadriceps at Different Positions of the Knee and Hip Joints in Human توانبخشي و فيزيوتراپي Rehabilitation پژوهشي Research <roundrect id="_x0000_s1026" style="MARGIN-TOP: 0.8pt; Z-INDEX: -1; LEFT: 0px; MARGIN-LEFT: 10.35pt; WIDTH: 459pt; POSITION: absolute; HEIGHT: 380.1pt; TEXT-ALIGN: left" arcsize="1496f" /><textbox style="mso-next-textbox: #_x0000_s1026" /><table cellspacing="0" cellpadding="0" width="100%"><tbody><tr><td style="BORDER-RIGHT: #ece9d8; BORDER-TOP: #ece9d8; BORDER-LEFT: #ece9d8; BORDER-BOTTOM: #ece9d8; BACKGROUND-COLOR: transparent"><div> </div></td></tr></tbody></table></textbox /><wrap anchorx="page" /></roundrect />خلاصهسابقه و هدف: فعاليت حركتي در عضلات ناشي از تداخل تحريكاتي است كه از سيستم عصبي مركزي و محيطي به نخاع مي‌رسند. اين تداخل، كه عمدتا در محل نورون‌هاي واسطة مشترك اتفاق مي‌افتد، باعث مي‌شوند كه فعاليت عضلاني به نحوي تنظيم گردد كه برآورده كننده نيازهاي حركتي باشد. تحريك آوران‌هاي گروه I وII در عصب پرونئال مشترك در انسان باعث يك رفلكس چند سيناپسي در عضلات چهارسرراني مي‌گردد. نورون‌هاي واسطه در اين مسير رفلكسي محلي براي تداخل مسيرهاي متعدد نزولي و صعودي ديگر مي‌باشند. در اين مطالعه حاضر الگوي تغييرات اين رفلكس در وضعيت‌هاي مختلف مفاصل زانو ‌و ران در ۱۴ داوطلب ‌مورد بررسي قرار گرفت.مواد و روش‌ها: تحريكات الكتريكي تك پالس با شدت 5/1 برابر آستانه حركتي در عضله قدامي ساق پا و طول موج ms2/0 به عصب پرونئال مشترك اعمال و پاسخ‌هاي حركتي در الكتروميوگرام عضله ركتوس فموريس همان سمت در حين انقباض ارادي و در وضعيت‌هاي مختلف مفصل زانو و ران ثبت گرديد. تغييرات اندازه رفلكس فوق در شدت‌هاي مختلف انقباض عضله ركتوس فموريس نيز در دو وضعيت o۱۸۰وo۱۳۰زانو مورد بررسي قرار گرفت.يافته‌ها: اندازة رفلكس‌هاي مشاهده شده افت بارزي را به دنبال خم شدن زانو و باز شدن مفصل ران از خود نشان دادند. هم‌چنين ارتباط مستقيم و معني‌داري بين شدت انقباض عضله ركتوس فموريس و بزرگي رفلكس در وضعيت باز شدگي كامل زانو مشاهده شد، در حالي كه اين ارتباط در وضعيت خميدة زانو وجود نداشت.نتيجه‌گيري: نتايج حاصل نشان‌دهنده وجود يك اثر مهاري قوي بر روي رفلكس فوق مي‌باشد كه احتمالاً ناشي از كشيده شدن و تحريك دوك‌هاي عضلاني در عضلات چهار سر راني مي‌باشد. اين اثر مهاري به نظر در سطح سيناپس ما قبل نهايي عمل مي‌كند.واژه‌هاي كليدي: رفلكس CPQ، خم شدن زانو، عضلات باز كنندة زانو، عضلات قدامي ساق پا The Role of Pretibial Muscles in the Motor Control of Quadriceps at Different Positions of the Knee and Hip Joints in Human K . This interaction, which occures at the site of convergence of the common interneurones, can modify the motor activities to meet the demands of the external requirements. Stimulation of group I and II afferents in common peroneal nerve (CPN) in human elicits poly- synaptic excitation of quadriceps motoneurones. The interneurones in this pathway are the point of convergence for many ascending and descending volleys. In this study, the gain modulation of this reflex at the positions of several knee and hip joints was investigated in 14 volunteers. Materials and Methods : Single 0.2ms pulses were delivered to CPN at the intensity of 1.5 ´ motor threshold in anterior tibials. The excitatory responses were recorded in ongoing EMG of voluntarily contracted ipsi-lateral rectus femoris at different knee and hip positions. The changes of the reflex at different intensities of contraction in rectus femoris at two knee positions of 180o & 130o, were also investigated. Results : The CPQ reflexes showed a significant attenuation with the knee flexion and hip extension (P £ 0.001, P £ 0.01). A positive significant correlation was found between the reflex magnitude, and the level of contraction at extended knee position, however at the flexed position this correlation was not significant. Conclusion : The results of this study indicated a strong inhibition of CPQ reflex, which is possibly raised from activation of stretch receptors in quadriceps muscles. This inhibitory effect is probably acting at premotoneuronal level. fa شيوع گواتر و يد ادرار در دانش آموزان 11-6 سالة شهر يزد در سال 1381 Prevalence of Goiter and Urinary Iodine in 6-11 Years Old Students of Yazd in the Year 2002 تغذيه پژوهشي Research <div class="Section1" dir="rtl">خلاصه سابقه و هدف: اختلالات ناشي از كمبود يد يكي از مهم‌ترين اپيدمي‌ها در دنيا مي‌باشد و در كشور ما نيز از سال‌هاي گذشته به عنوان يكي از اپيدمي‌هاي مهم بوده است. اينك از شروع برنامه يددار كردن نمك نزديك به 10 سال مي‌گذرد. براي ارزيابي وضعيت اختلالات ناشي از كمبود يد، شناسايي مناطق در معرض خطر و پايش و ارزشيابي برنامه هاي كنترل آن، مثل برنامة يددار كردن نمك، از اهميت ويژه‌اي برخوردار مي‌باشند. هدف از اين مطالعه تعيين شيوع كلي گواتر و ميزان يد ادرار دانش آموزان 11-6 سالة شهرستان يزد و نيز مشخص كردن وضعيت شهرستان پس از 10 سال اجراي برنامة يددار كردن نمك طعام است.مواد و روش‌ها: در اين مطالعه 1989 نفر دانش آموز ابتدايي(1367 نفر پسر و 622 نفر دختر) 6 تا 11 ساله از مدارس ابتدايي شهرستان يزد به روش نمونه‌گيري خوشه‌اي چند مرحله‌اي انتخاب و توسط دو پزشك آموزش ديده در ارتباط با معاينة غدة تيروييد دانش آموزان معاينه شدند. از يك سيزدهم كل افراد (140 نفر) نمونة ادرار و نمك مصرفي در منزل جمع‌آوري شد. يد ادرار به روش هضمي و يد نمك با استفاده از كيت تست سريع به صورت كيفي اندازه‌گيري گرديد.يافته‌ها: شيوع كلي گواتر در شهرستان 2/40، در دختران 41 و در پسران 9/39 درصد مشخص شد. از سوي ديگر شيوع گواتر درجة يك 7/38 (دختران 2/39% و پسران 5/38%) و درجة 2, 5/1 درصد (دختران 8/1% و پسران 4/1%) بدست آمد. تفاوت بين دختران و پسران از لحاظ آماري معني‌دار نبود. شيوع كلي گواتر در سنين 9/6-6 , 9/7-7 , 9/8-8 , 9/9 –9 و 11-10 سال به ترتيب 39، 5/37، 42، 40 و 1/42، شيوع گواتر درجة 1 به ترتيب 7/38 , 2/35 , 5/40, 8/38 و 9/39 درصد و شيوع گواتر درجه 2 در اين سنين به ترتيب 3/0 , 3/2 , 5/1 , 3/1 و 2/2 درصد بدست آمده است كه شيوع گواتر در سنين مختلف تفاوت آماري معني‌داري نيست ( 3/. = p). ميانگين كلي يد ادرار 122±258 ميكروگرم در ليتر، (در دختران 115±244 و در پسران 125±265 ميكروگرم در ليتر) بدست آمد كه تفاوت به لحاظ آماري بين دو جنس معني‌دار نبود. در مجموع 10 درصد از افراد يد ادرارشان كمتر از 100، 4/26 درصد در حد كافي، يعني 200-100 و 6/63 درصد بيش از 200 ميكروگرم در ليتر بوده است. 7/40 درصد از افراد گروه يد ادرارشان 300 ميكروگرم در ليتر و بالاتر بود. در كل 2/3 درصد از نمونه‌هاي نمك فاقد يد بوده است، 6/5 درصد در حد 15 پي.پي.ام و 2/91 درصد در حد 30 پي.پي.ام و بالاتر يد داشته‌اند. به اين ترتيب تقريباً 8/96 درصد از نمونه‌هاي نمك يددار بوده است.نتيجه گيري: نتايج اين بررسي نشان مي دهد، شيوع كلي گواتر، بخصوص گواتر درجة 2 بشدت در شهرستان يزد كاهش يافته است و به لحاظ يد ادرار، وضعيت نسبت به گذشته بسيار تغيير كرده است، به طوري كه نه فقط يد ادرار افزايش يافته، بلكه درصد عمده‌اي از افراد يد ادرارشان از محدودة ايده آل تعيين شده توسط سازمان جهاني بهداشت فراتر رفته است واژه‌هاي كليدي: يزد، كمبود يد، گواتر اندميك، يد ادرار، دانش آموزان ابتدايي </div> Prevalence of Goiter and Urinary Iodine in 6-11 Years Old Students of Yazd in the Year 2002. The surveillance of IDD may be implemented to: assessing the prevalence of IDD, identifying the high risk areas for intervention and monitoring and evaluateing IDD control programmes, as salt iodized programme. The aim of this study was to determine the total goiter rate (TGR), urinary iodine in 6-11 years old students in Yazd city 10 years after iodized salt program. Materials and Methods: In this study, 1989 primary school students (1367 boys and 622 girls) aged 6-11 were selected by multistage “probability proportionate to size” cluster(PPS) sampling method in Yazd city. Thyroid examination and goiter classification were made according to WHO standard protocol by two trained physicians. Urine and household salt samples were collected from one-thirteenth of the total population (140 individuals). Urinary iodine was determined by digestion method. Salt iodine was determined by rapid test kit. Results: TGR of Yazd city was 40.2% (Girls 41% and boys 39.9%). Prevalence of goiter grade 1 and grade 2 were 38.7% (39.2% for girls and 38.5% for boys), and 1.5% (1.8% for girls and 1.4% for boys), respectively. The rate of goiter was not significant between the two sexes). Age specific TGR at 6-6.9,7-7.9, 8-8.9, 9-9.9 and 10-11 age groups were 39%, 37.7%, 42%, 40% and 42.1%, respectively. Age spesific goiter grade 1 in this age groups were 38.7%, 35.2%, 40.5%, 38.8% and 39.9%, respectively. Age spesific goiter grade 2 in this age groups were 0.3%, 2.3%, 1.5%, 1.3% and 2.2% , respectively. TGR in this age groups were not significant(p=0.3). The mean of the urinary iodine was 258±122 μ g/l (244 ± 115 and 265 ± 125 μ g/l) in girls and boys respectivly, which were not significantly different. In general, 10% of Yazd study population had urinary iodine concentrations less than 100 μ g/l, 26.4% were in adequate range (100-200 μ g/l), 63.6% were higher than 200 μ g/l the urine iodine 40.7% of the 3rd group were higher than 300 μ g/l. In general, 3.2% of household salt samples did not contain iodine, 5.6% had 15ppm level, and 91.2% had above 30ppm level. Thus, approximately 96.8% of salts had iodine. Conclusion: The results of this study indicated that TGR, specially goiter grade 2, had significantly decreased in Yazd, and according to urinary iodine status, not only the mean of urinary iodine was elevated, but also was higher than ideal range of WHO. fa مقايسه پاسخ ضددردي مرفين بعد از برداشتن رحم و تخمدان در موش‌هاي صحرايي وابسته به مرفين Comparison of Analgesic Response to Morphine After Ovariohysterectomy in Morphine Dependent Rats فارماكولوژي Pharmacology پژوهشي Research <roundrect id="_x0000_s1026" style="MARGIN-TOP: 0.4pt; Z-INDEX: -2; LEFT: 0px; MARGIN-LEFT: 10.35pt; WIDTH: 459pt; POSITION: absolute; HEIGHT: 475.95pt; TEXT-ALIGN: left" arcsize="1496f" /><textbox style="mso-next-textbox: #_x0000_s1026" /><table cellspacing="0" cellpadding="0" width="100%"><tbody><tr><td style="BORDER-RIGHT: #ece9d8; BORDER-TOP: #ece9d8; BORDER-LEFT: #ece9d8; BORDER-BOTTOM: #ece9d8; BACKGROUND-COLOR: transparent"><div> </div></td></tr></tbody></table></textbox /><wrap anchorx="page" /></roundrect />خلاصهسابقه و هدف: داروهاي ضددرد اپيوييدي بهترين داروهايي هستند كه جهت تسكين يا درمان دردهاي جراحي مورد استفاده قرار مي‌گيرند. ولي مصرف مزمن داروهاي فوق موجب بروز تحمل در فرد مي‌شود. با اين حال ميزان نياز واقعي و پاسخ به مرفين پس از درد جراحي در افراد وابسته به مرفين دقيقاً مطالعه نشده است. لذا مطالعه حاضر بدين منظور صورت گرفت تا پاسخ ضددردي به مرفين پس از عمل جراحي اواريوهيستركتومي در موش‌هاي صحرايي وابسته در مقايسه با موش‌هاي صحرايي غير وابسته به مرفين مقايسه گردد.مواد و روش‌ها: مطالعه حاضر بر روي 36 سر موش صحرايي ماده بالغ صورت گرفت. موش‌هاي صحرايي ماده به چهار گروه 6 تايي كنترل- اواريوهيستركتومي، كنترل-sham، وابسته- sham و وابسته- اواريوهيستركتومي تقسيم شدند. وابستگي به مرفين به روش مصرف خوراكي مرفين از طريق آب آشاميدني به مدت 21 روز ايجاد گرديد. عمل جراحي اواريوهسيتركتومي پس از بي‌هوشي حيوانات با اتر و پس از شكافتن شكم و خارج نمودن تخمدان و رحم در قسمت بالاي سرويكس انجام گرديد. در تمام گروه‌ها پاسخ ضددردي توسط آزمون Tail flick قبل و پس از عمل جراحي اندازه‌گيري شد. سپس همين آزمون قبل و پس از تزريق مرفين (4mg/kg) از طريق زيرپوستي نيز مجدداً اندازه‌گيري شد. نتايج حاصل به صورت ميانگين پاسخ تأخيري و درصد حداكثر پاسخ ممكن (%MPE) بيان گرديد.يافته‌ها: نتايج تحقيق حاضر نشان داد كه ميانگين پاسخ تاخيري به محرك دردناك در آزمون Tail flick قبل و پس از عمل جراحي در چهار گروه موش‌هاي مورد آزمايش تفاوت معني‌داري ندارد. بدين معني كه آستانه حس درد در موش‌هاي صحرايي وابسته به مرفين (4/0±1/9ثانيه) تفاوت معني‌داري با گروه كنترل (25/1±1/10 ثانيه) ندارد. هم‌چنين تزريق مرفين موجب افزايش معني‌داري در ميانگين پاسخ تأخيري در كليه گروه‌هاي آزمايشي گرديد. مقايسه حداكثر پاسخ ممكن (%MPE) نشان ‌داد كه حداكثر اثرضددردي مرفين فقط در موش‌هاي صحرايي وابسته به مرفين پس از عمل اواريوهيستركتومي (8/11±7/48 ثانيه) به طور معني‌داري بيشتر از گروه وابسته-sham (5±6/19 ثانيه) بود.نتيجه‌گيري: مطالعه حاضر نشان داد كه اثر ضددردي مرفين متعاقب عمل جراحي اواريوهيستركتومي در موش‌هاي وابسته به مرفين و سالم تفاوت معني‌داري را ندارد. اما عمل جراحي اواريوهيستركتومي فقط در موش‌هاي وابسته موجب افزايش معني‌دار اثر ضددردي مرفين شده است.واژه‌هاي كليدي: درد پس از عمل جراحي، وابستگي به مرفين، پاسخ تأخيري<wrapblock /><rect id="_x0000_s1027" style="MARGIN-TOP: 648.65pt; Z-INDEX: 2; LEFT: 0px; MARGIN-LEFT: -25.65pt; WIDTH: 162pt; POSITION: absolute; HEIGHT: 18pt; TEXT-ALIGN: left; mso-position-horizontal: absolute; mso-position-vertical: absolute" strokecolor="white" /><wrap anchorx="page" /></rect /></wrapblock /> Comparison of Analgesic Response to Morphine After Ovariohysterectomy in Morphine Dependent Rats M. But due to the development of tolerance, chronic use of opioids limits their effectiveness. However, the real need and analgesic response to opioids in postoperative pain has not been determined in addicted patients. Therefore this study was performed, to evaluate the analgesic response to morphine in postoperative pain of ovariohysterectomy in morphine dependent rats, compared with the non dependents. Materials and Methods: The present study was carried out on 36 adult female rats. Animals were divided into four groups (6 in each group): Control– ovariohysterectomy, control – sham, dependent– sham and dependent – ovariohysterectomy groups. Rats became dependent to morphine by taking morphine in their drinking water for 21 days. Rats were anaesthetized with ether, and after incision of the abdomen, uterus and ovaries were removed above the cervix. Latency time was assessed by Tail flick test before and after the operation and after inducing morphine 4 mg/kg (sub cutaneously). Data were expressed as latency time and percent of maximal possible effect [%MPE]. Results: The results of present study showed that the mean latency time to noxious stimulus in tail flick test was not significantly different before and after the surgery in the four groups of animals Which indicated. that latency time in morphine dependent rats (9.1 ± 0.4s), was not significantly different from control rats (10.1 ± 1.25s) in tail flick test. Morphine injection increased the latency time in all of the groups significantly. The percent of MPE showed that the maximum analgesic effect of morphine in morphine dependent rats [48.7 ± 11.8s] after ovariohysterectomy was significantly higher than sham – dependent rats (19.6 ± 5s). Conclusion: The present study showed that there was not a significant difference in analgesic response to morphine after ovariohysterectomy in normal and morphine dependent rats. fa مقايسه نتايج دو روش درماني ايمي‌پرامين با تركيب ايمي‌پرامين و اكسي‌بوتينين در بچه‌هاي 12-6 سال مبتلا به شب ادراري غيرعارضه دار Comparison of the Results of Two Therapeutic Methods: Imipramine with Combination of Imipramine and Oxybutynin in Uncomplicated Nocturnal Enuresis in 6-12 Year Old Children ارولوژی Urology پژوهشي Research <div class="Section1">خلاصهسابقه و هدف: ايمي‌پرامين يكي از درمان‌هاي دارويي رايج در شب ادراري كودكان محسوب شده كه به طور متوسط 60-50% مؤثر است، هدف از اين مطالعه بررسي تأثير ايمي‌پرامين همراه با اكسي‌بوتينين در مقايسه باتك داروي ايمي‌پرامين در درمان شب ادراري غيرعارضه دار كودكان مي‌باشد. مواد و روش‌ها: ‌اين مطالعه يك كارآزمايي باليني است و در آن 80 كودك دبستاني 12-6 ساله مبتلا به شب ادراري بدون عارضه كه از ميان 2500 كودك دبستاني شهر رفسنجان از طريق پرسش‌نامه غربالگري شده بودند، به صورت تصادفي به دو گروه 40 نفري تقسيم شدند، يك گروه تحت درمان با ايمي‌پرامين توأم با اكسي‌بوتينين و گروه ديگر تحت درمان با ايمي‌پرامين و پلاسبو به مدت 3 ماه قرار گرفتند. بعد از اتمام دوره درماني و پيگيري در طول مدت درمان، موارد بهبودي و عدم بهبودي بررسي شد. يافته‌ها: نتايج اين پژوهش نشان داد كه ميزان بهبودي با ايمي‌پرامين و پلاسبو60% و ايمي‌پرامين توأم با اكسي‌بوتينين 95% بدست آمد كه از نظر باليني و آماري معني‌دار بود (00017/0=P).نتيجه‌گيري: بنظر مي‌رسد روش درمان دو دارويي ايمي‌پرامين و اكسي‌بوتينين در شب ادراري غيرعارضه‌دار كودكان، روش مؤثرتري از درمان تك دارويي ايمي‌پرامين باشد هرچند پيشنهاد مي‌شود درمطالعات بعدي، ميزان عود شب ادراري و عوارض جانبي دارويي درهركدام از متد فوق، مورد ارزيابي قرار گيرد. واژه‌هاي كليدي: شب ادراري، ايمي‌پرامين، اكسي بوتينين </div> Comparison of the Results of Two Therapeutic Methods : Imipramine with Combination of Imipramine and Oxybutynin in Uncomplicated Nocturnal Enuresis in 6-12 Year Old Children M. The main purpose of this research was to investigate the therapeutic effect of the combination of imipramine and oxybutynin compared to imipramine alone in uncomplicated nocturnal enuretic children. Materials and Methods: This clinical trial study was carried out on 80 primary school children (6-12 years old) with uncomplicated enuresis that were selected among 2500 school children with screening by questionnaire. The children were randomly divided into two groups (40 in each group). One group was treated with Imipramine in combination with Oxybutynin and the other group was treated with Imipramine and placebo for a period of 3 months. After the treatment period, the treated and untreated children were determined. Results: The frequency of cured with Imipramine and Oxybutynin therapy was 95%, and with Imipramine and placebo was 60%; therefore there was a significant difference between the two therapeutic methods, clinically and statistically (P=0.00017). Conclusion: The Result of this study showed that Imipramine and Oxybutynin treatment was more effective than Imipramine alone, although we recommend further studies to determine the rate of relapses and side effects of the above methods. fa تأثير تمرين ذهني قدرتي بر افزايش قدرت عضلاني حداكثر در زنان سالم 20 تا 30 سال The Effect of Mental Practice on Increasing Maximal Muscle Strength in 20-30 year old Healthy women توانبخشي و فيزيوتراپي Rehabilitation پژوهشي Research <div class="Section1">خلاصهسابقه و هدف: سال‌هاست كه تحقيقات در زمينه يافتن راه حلي جهت درمان ضايعات عضلاني از جمله ضعف و آتروفي كه از عواقب ناشي از بي‌حركتي هستند، در حال انجام است. درمان‌گران همواره به دنبال روشي جهت اجتناب از اين ضعف و آتروفي، خصوصاً در زماني كه انجام حركات فعال ممنوعيت دارد، مي‌باشند. به نظر مي‌رسد تمرين ذهني، راه‌گشاي حل اين معضل باشد. هدف از انجام اين تحقيق، بررسي ميزان تأثير اين نوع تمرينات بر افزايش قدرت عضلاني مي‌باشد.مواد و روش‌ها: تحقيق حاضر به صورت مداخله‌اي بر روي 30 نفراز زنان (30-20 سال) سالم كه به صورت تصادفي در دو گروه آزمايش و كنترل قرار گرفتند، انجام شد. افراد گروه آزمايش به مدت سه هفته تمرين ذهني قدرتي كشش آرنج انجام ‌دادند، و افراد كنترل در همان دوره زماني به انجام تمرينات ذهني رياضي ‌پرداختند. ميزان گشتاور عضلاني اكستنشن آرنج قبل و بعد از مداخله با استفاده از دينامومتر Isostation B200 مورد سنجش قرار ‌گرفت. نتايج با استفاده از آزمون ناپارامتري ”من- ويتني“و ”ويلكا كسون“ مورد بررسي آماري قرار گرفت.يافته‌ها: انجام تمرين ذهني قدرتي باعث افزايش 50 درصدي در گشتاور ايزومتريك اكستنشن آرنج اندامي كه روي آن مداخله انجام گرفته بود، شد، در حالي كه افراد گروه كنترل تنها به ميزان 7 درصد افزايش قدرت نشان دادند.نتيجه‌گيري:به نظر مي‌رسد از تمرين ذهني قدرتي مي‌توان جهت حفظ و يا افزايش قدرت عضلاني سود جست. پيشنهاد مي‌شود درمان‌گران از تركيب اين روش جديد با ساير روش‌هاي معمول در برنامه‌هاي توان‌بخشي خود استفاده نمايند.واژه‌هاي كليدي: تمرين ذهني، قدرت عضلاني، تصويرسازي </div> The Effect of Mental Practice on Increasing Maximal Muscle Strength in 20-30 year old Healthy women R. Clinicians are looking for a way to prevent these weaknesses and atrophies specially, when active movements are forbidden. It seems that mental practice can be a good answer to this problem. The aim of this study was to identify the efficacy of this kind of exercises on increasing muscle strength. Materials and Methods: This study was performed on 30 young healthy females (20-30 years old), who were randomly divided into case and control groups. The case group performed imaginary maximal contractions of the elbow extensors for 3 weeks while the control group performed mathematical exercises at the same period of time. The amount of muscle extension torque of elbow was determined by Isostation B200 Dynamometer, before and after the intervention. The statistic analysis was performed with non-parametric tests of Mann-Whitney and Wilcoxon. Results: Mental practice of strengthening tasks produced 50 percent increase in the isometric torque of elbow extension while control group showed only 7 percent increase in their torques . Conclusion: It seems that we can use mental practice to increase or keep the muscle strength. It is suggested that physiotherapists combine this new method with the other common methods in their rehabilitation programs. fa فراواني موارد مثبت HBsAg در سرم زنان باردار رفسنجان در سال 1382 Frequency of HBsAg Positive in Pregnant Women Rafsanjan in the Year 2003 روانپزشكي Psychiatry پژوهشي Research <div class="Section1">خلاصهسابقه و هدف: براساس برآورد سازمان جهاني بهداشت در حدود 350 ميليون نفر از جمعيت جهان به ويروس هپاتيت B آلوده هستند و سالانه در حدود 2 ميليون نفر از عوارض هپاتيت B، جان خود را از دست مي‌دهند. هدف اين تحقيق، تعيين فراواني موارد مثبت HBsAg در سرم زنان باردار مراجعه كننده به زايشگاه نيك نفس شهر رفسنجان سال 1382 بوده است.مواد و روش‌ها: اين مطالعه يك مطالعه توصيفي مقطعي درسال 1382 مي‌باشد، كه تعداد 600 نفر زن باردار كه از فروردين تا تير ماه سال 1382 جهت زايمان به زايشگاه نيك‌نفس شهر رفسنجان مراجعه مي‌كردند، انتخاب شدند و نمونه سرم آنها توسط آزمايشگاه به روش اليزا از نظر HBsAg مورد بررسي قرارگرفت، سپس پرسش‌نامه‌اي كه توسط پژوهشگران تدوين شده بود، تكميل گرديد. داده‌ها با نرم‌افزار SPSS 11.5 به وسيله آزمون X2 و فيشر آناليز گرديد.يافته‌ها:براساس نتايج اين تحقيق، سرم خون 8 مورد (3/1%)، ]دامنه اطمينان95%، (35/1%-26/1%)[ از600 مورد زنان باردار تحت مطالعه، از نظر HBsAg مثبت بودند. ميانگين سني افراد تحت مطالعه3/5±2/26سال، ميانگين تعداد حاملگي41/1±14/2بود.ازنظرسابقه اعتيادتزريقي همسر، خالكوبي، سابقه تزريق مواد مخدر، غيربومي بودن، در گروه HBsAg مثبت با درصد بالاتر نسبت به گروه HBsAg منفي اختلاف معني‌دار آماري وجود داشت.نتيجه‌گيري:با توجه به ميزان فراواني هپاتيت B در اين مطالعه، نياز به آموزش همگاني، به ويژه آموزش هنگام ازدواج و نيز آموزش حين مراقبت‌هاي زمان بارداري، مي‌باشد تا گام موثري در كاهش ابتلا زنان باردار به عفونت هپاتيت B برداشته شود.كليد واژه‌ها: فراواني آنتي‌ژن سطحي ويروس هپاتيت B، زنان باردار، رفسنجان </div>  Frequency of HBsAg Positive in Pregnant Women Rafsanjan in the Year 2003   Z. It is estimated that about 2 million people die from HBV complications such as cirrhosis, hepatocellular carcinoma every year. The aim of this study was to determine the frequency of HBsAg+ among women referring to Niknafs Delivery Center of Rafsanjan (NDCR), from April to July 2003. Materials and Methods: This was a cross- sectional descriptive study evaluating 600 pregnant women referring to NDCR from April to July 2003. Serum samples were checked for HBsAg+ by ELISA 0/13. Data were recorded and analysed by SPSS 11.5 Soft Ware using x 2 Fisher tests. Results: According to the results of this study from 600 pregnant women 8(1.3%) were HBsAg+ (1.26%-1.35%,CI 95%). The mean age was 26.2 ±5.3 and the mean number of pregnancies were 2.14±1.4. There were significant differences in the history of husband’s IVD-usage, tatooing, previous IVD usage, not being native, between the two groups (p<0.05). Conclusion: Considering the frequancy of HBsAg+ in this study in order to reduce the infection and morbidity rate of HBsAg+ in women, mass education at the time of marriage and prenatal period is needed.
